Fig.1 Pharyngeal protector is inserted through the pharynx into oesophagus. The bevelled edge faces the posterior tracheal wall anteriorly.
Fig.2 Tracheo-oesophageal puncture done by inserting trocar and cannula through the traceo-oesophageal party wall into pharyngeal protector. Puncture is done 5 mm from tracheo-cutaneous junction.
Fig.3 Trocar is removed. Guide wire is passed through the cannula into pharynx. Valve is held on retainer slit at the tip.
Fig.4 Guide wire is withdrawn and the outer flange of the valve is pulled out into trachea.
